REGISTER WITH THE PRACTICE ?

All new patients will be provided with registration forms and health questionnaires on requesting to join the practice; these need to be completed and returned to the surgery before we are able to register you as a patient. Once the completed forms are received, all patients over the age of five years will be offered an appointment to see our practice sister for an introductory health check. This also gives us the opportunity to meet you and your family and tell you about the practice.

Patients whose application to join the list is accepted have a right to express a preference to receive services from a particular doctor but, whilst we will endeavour to comply, it might not always be possible and if this were to be the case, an explanation would be offered.
