DO I....
OBTAIN
A REPEAT PRESCRIPTION ?
If your doctor has
authorised a repeat prescription, you may submit a written request or a computer
generated request slip to obtain your medication. Your requests can be handed
in, posted to the surgery or faxed. Yor prescription will be available for collection two days following the practice having received your request. If you wish it to be posted back to you please ensure that
you enclose a stamped addressed envelope. Please read your prescription counterfoil
each time you receive as new one to check that the details are correct. There
may be a message printed on it asking you to see the doctor or nurse before your
next prescription can be issued. The following chemists call at the surgery on
a daily basis:
- Haslucks Green Pharmacy, Haslucks Green Road.Lloyds Pharmacy, Tanworth Lane.
- Sainsbury's Pharmacy, Stratford Road.
If you wish to collect your medication direct from one of these chemists, please
mark your prescription request with the name of the chemist you intend to collect
your medication from. Please allow at least three working days from the time your request arrived before collecting your medication from the chemist.
Sick Notes
For all illnesses
lasting six working days or less, a sick note from your doctor is not required.
Instead, on returning to work complete a self-certification form, which is available
from your employer. If a note for a private insurance scheme is required, then
a charge will be made as this falls outside the NHS system.
